What if I need surgery or admitted to the hospital

What if I need specialty care?
Once your health care provider has determined you need referred to a specialist, the Referral Coordinator will contact the appropriate specialist and schedule your appointment. You will be notified of your appointment date and time. You will also be encouraged to apply for that health care providers "Charity Care" program which will assist in the cost of the specialty care for low income patients. I think I might be pregnant, what should I do?
Call 217-788-2300 to schedule an appointment if you think you might be pregnant. CCHC offers pregnancy testing, pre-natal care and post-natal care, and encourages you to start pre-natal care as soon as possible in order to help ensure the health and safety of you and your baby.
How much will it cost to see a doctor/dentist?
All services are rendered based on a sliding fee scale. Your cost is based on your income. Many uninsured patients pay only $20 on the day of their visit. Please refer to the sliding fee scale posted here and at each reception window.
What if I need labs or x-rays?
CCHC works with the local hospitals for lab and x-ray services. You will be instructed at the time of your appointment where to go for labs and x-rays if needed.
I need to see a doctor today, what should I do?
Please call 217-788-2300 and request a same day appointment. You will be scheduled in the next available appointment slot. CCHC also accepts walk-ins. You may walk in and request an appointment at any time and you will be scheduled for the next available time slot. If CCHC is not able to get you into see a physician today, a time that is convenient for you will be scheduled.
I need help applying for Medicaid.
CCHC has employees dedicated to assisting people apply for Medicaid and other programs. If you are not currently receiving Medicaid benefits and are not sure how to apply or if you are eligible, CCHC staff will be happy to help. How do I get prescription refills?
CCHC electronically prescribes all medications. Please contact your pharmacy directly to request a medication refill a few days before you prescription runs out.
I lost my job and health insurance, can I still see a doctor?
Yes you can. Please call 217-788-2300 to register as a patient and you will be scheduled to see a physician as soon as possible.
What is a community health center
A community health center, or federally qualified health center (FQHC), is a medical facility which provides high quality, affordable primary health and dental care to the uninsured and underinsured populations of the community. What do I need to bring to my appointment?
At the time of registration, please bring identification and proof of income (please note, income will be verified through the IRS). If you are a registered patient coming in for an appointment, please bring your most recent insurance card (if applicable), or be prepared to pay your minimum payment if uninsured, as well as any changes in your medical history and a list of current prescription medications.
